Died to Cyan.
Then found no part of items in pit.
Now sometimes found lost items 'weared' on character ('under' already weared items), so when i remove ring, there appears to be 'lost' ring on same finger.

Could you please prodive more details? Which class/race are you playing? Does it happen only when you die? Only from PK, or mobs too?
